Health surveys being sent out this month In January, February and March of 2018, nearly 1,200 adults living in Wyandot County will be receiving a health survey by mail. Adults are randomly selected to receive this survey, which is mailed from Toledo and will arrive in a large colored envelope containing a survey booklet and a pre-addressed, pre-stamped envelope to return the completed survey.
