Wyandot Memorial accepting Paramount Wyandot Memorial Hospital signed a contract with Paramount as a participating provider for Wyandot County residents purchasing health insurance through the individual marketplace. Health Alliance surveys to be sent out in mail In January, February and March of 2018, nearly 1,200 adults living in Wyandot County will be receiving a health survey by mail. Adults are randomly selected to receive this survey, which is mailed from Toledo and will arrive in a large colored envelope containing a survey booklet and a pre-addressed, pre-stamped envelope to return the completed survey.
