Level 1 Operation $140.00 + Mandated State Fee
Level 2 Operation $158.00 + Mandated State Fee
Level 3 Operation $310.00 + Mandated State Fee
Level 4 Operation $396.00 + Mandated State Fee
Food Service/Retail Food Establishment Operations Over 25,000 Sq. Ft.
Level 1 Operation $206.00 + Mandated State Fee
Level 2 Operation $217.00 + Mandated State Fee
Level 3 Operation $790.00 + Mandated State Fee
Level 4 Operation $838.00 + Mandated State Fee
High Risk Mobile Food Service Operation License Fee $172.00 + Mandated State Fee
